Pathways by Molina is seeking an innovative and purpose driven individual to join our team as an Enhanced Direct Care Worker.
An employee in this position is responsible for the direct care and supervision of a consumer or consumers participating in residential services. An employee in this position typically works in a 1:1 or similar supervision-intensive ratio with consumers who require enhanced support because of behavioral or psychiatric needs. Typically these consumers require a more comprehensive plan to be implemented than traditional consumers. Work assignments are varied in nature and are performed under the functional supervision of the Enhanced Residential Program Supervisor. Additional clinical supervision may be provided by the Behavioral Support Services Director for consumers who require more comprehensive behavioral planning or the Director of Nursing for consumers requiring a more intensive level of medical support.
• Participates in the development and implementation of the Individual Service Plans which require activities at the residence or in the community with single or multiple consumers who are ambulatory and non-ambulatory. Additionally, may participate in the development of enhanced plans to better support the behavioral and/or psychiatric needs of the consumer. • Collaborates with the consumer, their family, and their team for the purpose of developing a behavior or medical support plan that includes positive practices. • Participates in the collection of data for comprehensive functional assessments of presenting issues; such as aggression, self-injurious behavior, law offending behavior (sexual or otherwise). • Completes documentation related to behavior support plans including the tracking of frequency and duration of behavioral episodes as well as the antecedent and consequences as outlined in the behavior support plan. • Implements enhanced behavior modification plans that utilize positive strategies to support the consumer, based on functional behavioral assessments. • Ensures that each individual participates in social/leisure/recreational activities based on individualized plans which may require both activities in the program or in the community with single or multiple consumers who are ambulatory and non-ambulatory. • Provides social, emotional, and environmental supports to ensure the happiness, health, and safety of the consumers; requires the use of Crisis Prevention Intervention techniques including de-escalation techniques as outlined in the Behavior Support Plan and/or Restrictive Procedure Plans; physical interventions may be required in emergency situations where consumer/staff safety is impacted. • Provides support to the consumer(s) in activities of daily living such as personal/oral hygiene and appropriate dress; may require the transfer of non-ambulatory participants using mechanical apparatus or approved lifting/transfer techniques. • Performs chore duties and assists/supervises the consumers in performing these duties to ensure that the facility is clean, safe, and secure at all times. Duties include, but are not limited to, such tasks as vacuuming; sweeping; dusting; doing dishes; stocking groceries/supplies; preparing meals/snacks; cleaning appliances, countertops, and tables; doing laundry; and snow removal. • Prepares and completes required documentation in compliance with payer regulations, company standards, and the policy and procedure manuals. • Dispenses medication prescribed for the consumer in accordance with the approved ODP medication administration procedures, company standards, and the policy and procedure manuals. • Participates and provides input as needed in the assessment process to identify consumer strengths and current needs. • Encourages consumers in the acquisition of skills to achieve a greater level of independence. • Participates in required training sessions and meetings. Participates in additional trainings as assigned to maintain an appropriate level of knowledge pertaining to the changing psychiatric needs of the individual. • Reports concerns of programmatic or consumer-related nature to the Enhanced Residential Supervisor or other supervisory level as appropriate and in accordance with the chain of command.
REQUIRED SKILLS, KNOWLEDGE, AND ABILITIES:
• Knowledge of the basic principles of applied psychology, behavioral analysis, and related behavioral sciences. • Ability to implement a behavior support plan including the recognition of antecedents to problematic behaviors, the ability to de-escalate behaviors using authorized techniques, and implementation of consequence per the approved behavior support plan. • Ability to prepare clear and concise reports and communicate effectively, both orally and in writing, to the Enhanced Residential Supervisor and/or other team members. • Ability to develop and maintain a positive and understanding attitude toward the consumers. Ability to make sound judgments regarding the health and welfare of the consumers. • Ability to respond to emergency situations appropriately including the facilitation of emergency physical intervention techniques. • Ability to work as a member of a team cooperatively and ethically to ensure that the program is a safe, happy, and clean environment for all consumers. • Ability to communicate clearly with other staff members and follow instruction and direction in the performance of job tasks. • Ability to establish effective working relationships with co-workers, supervisors, families, and others who comprise the consumer's ISP team.
• Post-secondary education in a human service related field and/or be a licensed medical professional in the state of Pennsylvania or • An Associate's Degree or 60 credits in a related field and 4 years field experience OR • A Bachelor's Degree or 120 credits in a related field and 2 years field experience OR • Five years behavioral/psychiatric/IDD care field experience
