Responsible for providing comprehensive case management, health, and psychosocial education to individuals at Greenville Primary Care with the goal of linking patients to appropriate medical and social services.
Bachelor Degree required in social work or social work related field.
Masters Degree preferred in social work or social work related field.
Bi lingual Strongly preferred
Experience and knowledge of entitlement programs, and community resources is preferred.
Demonstrates the ability to function as a member of an interdisciplinary team.
Experience working with urban populations of multiple ethnic and cultural backgrounds
Good oral and written communication skills
