This position is responsible for the coordination of psycho-social care of patients and their support systems. The Social Worker is an integral part of the healthcare team's efforts in screening, assessment of needs, planning of post hospital care, implementing activity to assist patient and families, and evaluating outcomes of patients at high risk.
This position also includes research of appropriate resources for the population of this health-system and creating resource material to aide other healthcare team members in the discharge plan. Data collection, documentation mandates, and communications are of great importance in effective patient and family interactions and continuum of care.
Summarized tasks include: D/C plan; funding issues; psycho social referrals [DME, HHC, adoptions, psych, etc.]; placements and transfers; crisis intervention for grief, loss, neglect, and abuse; and identification of benefits and provider information
Licensed to practice Social Work in the State of Texas
Must have some experience in healthcare, either active hospital social work or extended academic clinical rotation
Must be graduate of an accredited university licensed for social work degree
Basic Computer Skills
