MSW will provides direct social work services to patients and families in an interdisciplinary team approach. Completes psychosocial assessments of patients, provides supportive emotional intervention, and develops plans for optimal recovery and safe transitions from the hospital. Manages complex discharge plans, builds and maintains relationships with internal/external resources, and refers to community agencies and entitlement programs to address, complex post- acute needs. Understands all stages of growth and development to assess each patient's age specific needs to provide individual treatment and care. Appreciates the social determinants of health that impair patients from maintaining wellness in the community.
Position requirements: Master's of Social Work degree. NJ State license required by NJ State Board of Social Work Examiners. Licensed Social Worker (LSW), or licensed clinical social worker (LCSW) required. Minimally one year of experience in a hospital setting required. Three (3) years of experience in a hospital setting preferred.
