This position has accountability for the direct provision of member health education, skill development, and ongoing support for members identified to participate in Seaside disease management programs (DMPs). The HEQ Coordinator reviews all member information available in care management and authorization databases and completes all relevant assessments in the case management platform —accessible to the member's care team. Conducts annual member health assessment and develops health education materials and programs based on health assessment data. Interfaces with the members, case managers and providers as required by protocols and appropriate to care planning and care outcomes.
* Bachelor's degree in Nursing, Health Science or other related field, required.
* Master's degree in Health Education, Public Health, Social Work or other related field, preferred.
* One (1) year of disease management or health coaching experience, preferably in a managed care environment
