Foundation Counseling and Wellness is seeking two Licensed Independent Clinical Social Workers, Adult and Lifespan trained, Masters Level Licensed Alcohol and Drug Counselor preferred, to join their growing behavioral health services team. These are outpatient positions serving patients who are part of Foundation Medical Partners primary care system. Clinicians will provide individual, couples and/or family therapy in a friendly professional environment. The clinician will also collaborate with the primary care provider, the embedded care coordinator and other specialty providers as appropriate.
