We are looking for a Licensed Clinical Social Worker that can elevate our patient's need by providing compassion, assessing the psycho-social of a patient and family needs and attains emotional and financial resources for patients being served by cCARE. This person will provide direct services including individual (including youth), couple, family and group therapeutic services to patients, including assessments, treatment plans and discharge planning.
Responsibilities/Essential Functions (other duties as assigned):
Effectively collaborates with physicians and patient care staff to meet the psycho-social needs of patients and family members.
Provides excellent case management support and assigns tasks to other team members or external resources as appropriate.
Completes documentation in the electronic medical record according to cCARE’s’ Standards.
Maintains current knowledge on community and financial resources for appropriate patient and family assistance and / or referral. Refers patients to local support groups and/or resources; Provides assistance with forms completion to obtain available services.
Effectively manages and maintains patient library and available resource materials including grants, co-pay assistance and insurance. Helps patient to complete MediCal forms.
At base location, facilitates weekly support groups for patients, family and caregivers. Develops and implements other beneficial educational programs / seminars for patients, family members and caregivers, as appropriate.
Meets or calls all new Radiation/Chemotherapy patients and follows up with these patients after last treatment.
Arranges and maintains facility calendar properly scheduling and coordinating meetings with pharmaceutical representatives and appropriate staff members.
Assists in the proper planning and organization and attendance of special events to increase community awareness of the Practice. In conjunction with Community Relations/Business Development, determines need for public outreach programs, including educational opportunities. (i.e., cancer screening, awareness, etc.) Will travel to cCARE sites for special events.
Effectively coordinates with Human Resources to provide intervention and support for appropriate staff members as needed. Mentors peers and health care staff in issues related to social work practice.
High School Diploma or equivalent required
Master’s degree in Social Work from an accredited educational institution required
Clinical Social Worker license is required and in good standing in State of California.
Bilingual capacity with ability to provide services in English and Spanish
Minimum of one years’ experience in a clinical setting with knowledge of Social Work Code of Ethics and mandated reporting requirements
Working knowledge of Clinic policies and procedures
Strong clerical/administrative skills including personal computer skills
Ability to make routine decisions and determine proper action based on experience within the limits of policy and
Current CPR certification required
Knowledge of computer/telephony support, preferably in a health care environment
Strong customer service background, preferably in health care environment
Excellent verbal communication skills
Competence with computer processing functions and other standard office equipment
Ability to manage and prioritize multiple tasks
Ability to calmly and professionally resolve customer issues with diplomacy and tact
Ability to work independently with minimal supervision
Strong organizational skills
