ChristianaCare is looking for an experienced, licensed Social Worker to assist patients in the Swank Center for Memory Care and Geriatric Consultation!
We are Delaware's first and most comprehensive outpatient office for patients with memory disorders and their families. Approximately 18,000 people across Delaware are coping with Alzheimer's disease or a related disorder. The Swank Center for Memory Care and Geriatric Consultation offers these patients and their families essential support, education and guidance from diagnosis through treatment. The Swank center sees close to 5 000 patients a year.
Social work support is a vital aspect of the care we deliver to patients at the Swank Center. The role of a Swank Social worker includes, but is not limited to the following:
Works with patients and their family/other members of the health care team to provide psychosocial support and education regarding availability and limitations of resources in the community.
Gathers and assesses information regarding the patient's physical needs, mental status, family support system, financial resources to determine goals that are designed to meet the client's needs that have been identified through assessment.
Monday - Friday - 8:30am -5:00pm

Education and experience requirements:
Master's degree in Social Work from an accredited graduate school of Social Work.
LCSW preferred, with two years of social work experience in a medical setting.
