Position Summary This position requires the full understanding of, and active participation in fulfilling the mission of Saddleback Memorial Medical Center (SMMC) and Memorial Care Health Systems (MHS). It is expected that the employee demonstrate behavior consistent with the core values of SMMC and MHS. The employee shall support SMMC's strategic plan and goals and direction of the performance improvement plan. The employee will also be expected to support all organizational expectations including, but not limited to: Customer Service, Patient's Rights, and Confidentiality of Information, Environment of Care, and Memorial Care initiatives. To provide clinical services to the palliative and terminally ill patient and family, to assist them to cope with the emotional, financial and practical demands of the illness and the dying process. Responsible for serving populations with a variety of clinical needs particularly psychological stress related to health impairments, serios illness. trauma and death and dying. Based on overall assessment, makes appropriate community referrals for follow up as needed and provides interventions designed to minimize patient stress and maximize coping skills.
