Summary:
Responsible for the timely scheduling, completion, and transmission of MDS. This position coordinates the assessment of subacute unit residents. Provides patient care/services for all stages of the lifespan served including adults and geriatrics.
Education:
  • Graduate from an accredited program leading to Registered Nurse licensure
Licenses/Certification:
  • Current RN license in the State where employed. CPR required or ability to obtain within probationary period. ANAAC or similar certification preferred.
Experience:
  • Two or more years of experience in long term care, at least one year's experience working with Minimum Data Set entry. Current clinical experience in the acute hospital setting one-year preferred.
