**Amended to correct hours** The Bay Pines VA Medical Center is a general medical and surgical facility, classified as a Level 1A Complexity. This position is located in Social Work Service of the Bay Pines VA Healthcare System. Incumbent will provide services at the Bradenton Community Outpatient Clinic. The duties and responsibilities are carried throughout the HealthCare Center including all clinical and other patient care areas involved with the services. The incumbent independently performs case finding functions and outreach, identifying and screening patients for psychosocial needs. The incumbent must possess the knowledge and ability to independently implement treatment modalities, provide counseling or psychotherapy for individuals, families and groups in the outpatient mental health clinic. Social Workers must practice within the bounds of their license or certification. For example, some states may require Social Workers providing psychotherapy to have a clinical level of licensure. The incumbent has the ability to work with patients and families who are experiencing a basic emotional, medical, and social problems, utilizing individual, group, and family counseling skills. The incumbent has the ability to assess the psychosocial functioning and needs of patients and their family members, and to formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills, and assistance needed. The incumbent has the ability to establish and maintain effective working relationships with clients, staff, and representatives of community agencies. He/She is able to communicate effectively, both orally and in writing, with people from varied backgrounds. The incumbent serves as liaison to community agencies and networks with agency representatives. The incumbent independently identifies high-risk patients and provides case management services. The incumbent provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members. Has working knowledge and experience in use of medical and mental health diagnoses, disabilities and treatment procedures.
