Social Worker - MSW

We have a full time Social Work position open at the University of Minnesota Masonic Childrens Hospital supporting our Maternal Child Health team. Social workersinitiate services with families being seen in the U of M MaternalFetal MedicineClinic, Antepartum Unit, Labor & Delivery, Postpartum Unit or the NICU. Patients have the same social worker across the continuum of care to optimize support, enhance relationship building, and provide continuity.

The Social Worker provides a wide range of clinical social work services and interventions to assist individuals, families, groups or communities to restore, enhance or protect their capacity for productive social functioning and coping with the consequences of their illness and health care status. They are able to assume independent practice and takes a leadership role in patient care, program and professional issues that they confront in their daily practice with minimal supervisory guidance.

Schedule:

Every Mon/Tues and Thurs/Fri as well as every other Saturday.

• Complex psychosocial needs
• Mental health concerns or current diagnosis
• Substance use concerns or current use
• Subsequent pregnancy after a fetal loss
• Anticipated NICU admission
• Child Protection Services (CPS) involvement
• Maternal Complex Care Conferences
• Positive toxicology screen for mom and/or baby
• Adoption and surrogacy
• Fetal loss and Perinatal Palliative Care
• Edinburgh score >10 AND any affirmative answer to question #10 (self harm
• Attend Rounds weekly
• Arrange and facilitate care conferences (family and staff) as needed
• Lead NICU Fellow Death Debriefings
• Present to Volunteers at Parent to Parent Meeting
• Attend NICU Collaborative Practice Meetings
• Attend NICU Mortality and Morbidity Conferences

Other responsibilites include:

• Plans and implements treatment and transition care plans based upon a psychosocial assessment that is integrated into an interdisciplinary care plan
• Provides a range of social work interventions that include casework, counseling crisis intervention, case management, and group work methods in assisting patient and families reach their goals. Identifies and recognizes rapidly changing situations and quickly intervenes to prevent or resolve the emotional crisis, and increased LOS, or unnecessary admissions
• Arranges, mobilizes, utilizes, and may coordinate a comprehensive range of community resources which most effectively meet the identified patient and family needs
• Takes leadership role in facilitation of interdisciplinary communication and guides team members interactions with patient/families experiencing complex psychosocial issues
• Formally and informally educates patient/family and health care team members regarding the social work role, pertinent patient care issues, and population specific information
• Serves as a preceptor or field instructor for social work graduate and undergraduate training as appropriate to their skills related to a range of psychosocial issues and interventions
• Serves as patient advocate and liaison with physicians, families and community to ensure continuity of care
• Documents patient and family assessment, history, diagnosis, treatment and transition care plan, and progress, in the medical record
• Identifies and participates in CQI, research and program evaluation initiatives that focus on improving health care services at a service, program. Or department level
• Demonstrates knowledge and compliance with Fairview and Social Work Services standards, policies, and procedures
• Complies with the legal and regulatory laws, reporting and practice standards within the practitioners scope of practice
• Meets all role-related and age-specific competencies

Required:

• Master's degree in Social Work with medical experience
• Current Minnesota Social Work Licensure

Preferred:

• One or more years of social work experience
• LGSW required-prefer LICSW
• Previous Maternal Child Health experience
