The Senior Social Worker assigned to the Perry Point Community Living Center (CLC) has specific knowledge of medical diagnoses and the needs of and services required for individuals with these conditions. In addition, the incumbent has specific knowledge regarding the needs of the advanced elderly, cognitively impaired individuals, and maintains a current, productive network referral resources to address the needs of these populations. The VAMHCS Community Living Center Senior Social Worker practices at the advanced level by providing comprehensive social work services to Veterans and their families. This social worker provides social work services to Veterans receiving care in the Perry Point VA Community Living Center. He/she is a member of the health care team and assesses and treats the psychosocial problems that impact on the ability of Veterans to make adjustments to health care issues. The incumbent has a broad and comprehensive knowledge of VA and community resources and uses a variety of interventions in the amelioration of medical conditions. Additionally, the incumbent serves as the Difficult Discharge Planning Consultant to other social work staff in the Community Living Centers (CLC) throughout the VAMHCS. The incumbent works on a campus where he/she is the only staff member functioning in this type of social work role. The incumbent serves as the subject matter expert for complex issues that arise in the CLC environment in the on-site absence of the supervisor.
