This position is responsible for the delivery of Whole Health Coaching and skill development of the local Medical Center's Whole Health Coach program, including provision of care through coaching/promotion/education regarding Whole Health Coaching to Veterans and staff. The position is integral to supporting Whole Health Coaches and Veterans in navigating the Whole Health Journey, specifically through the Pathway, wellbeing, and Whole Health clinical care elements of the Whole Health System. This position is responsible for the delivery of Whole Health Coaching and skill development of the local Medical Center's Whole Health Coach program, including provision of care through coaching, promotion and education regarding Whole Health Coaching to Veterans and staff. The position supports Whole Health Coaches and Veterans in navigating the Whole Health Journey, and assists in the collection of whole health qualitative data from Veterans and creation of reports analyzing effectiveness of health coaching. Duties include, but are not limited to: Contact eligible participants for enrollment and explain the health coaching process. Provide health coaching for Veterans in order to reduce or eliminate high-risk behaviors while increasing healthy ones. Conduct interventions for Veterans using one-on-one, couple or group modalities in outpatient and inpatient settings. Conduct Whole Health Facilitated Groups; participate in MOVE, smoking cessation, diabetes management, hypertension clinics, mental health and other inpatient and outpatient groups as appropriate. Provide follow-up care through telephone clinics or secure messaging between face-to-face appointments. Actively guide Veterans in the exploration of core elements of personal health planning through individual and group based programs. Encourage and assist Veterans with the adoption of habits that are conducive to a higher quality of life and in alignment with the Veteran's goals. Identify habits and behaviors that are harmful to quality of life and related to chronic illness. Develop treatment plans focused on behavioral interventions based on current scientific knowledge and research that improves quality of life and reduce risk for chronic disease or that reverse or slow progression of chronic disease. Review current health behaviors and develop customized treatment plans. Coordinate care with both primary and specialty care providers. Utilize motivational interviewing and health coaching techniques and process Develop trust with Veterans Maintain education and knowledge regarding lifestyle (habits and behaviors) and clinical outcomes, including behaviors that improve Veterans' health and well-being Program Development duties include, but not limited to: Develops information for Veterans about the Whole Health Group and Individual Programs. Conduct outreach activities Provide educational opportunities Collaborate with external stakeholders Develop and conduct onsite educational series Serve on Healthcare System, VISN and/or National VA committees or task forces relating to Whole Health or Health Coaching Participate in leadership roles on local, state and/or national professional organizations. Assist with data management analyses, special studies and reports Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Selection at a lower grade level does not guarantee promotion to the full performance level.
