This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Learn more The full time permanent GS-12 Senior Social Worker serves in the Palo Alto, California in the Social Work Service Line of the VA Palo Alto Health Care System. This position serves as a medical float social workers in the Medicine Section and ensures coverage and provision of social work services on a daily basis. Major duties include, but are not limited to: Provides comprehensive social work coverage and direct patient care services to multiple inpatient and outpatient medical programs. Evaluates and provides complex, multiple services to individuals and families with a variety of social, emotional and health related problems. Works in collaboration with a multi-disciplinary team on a fast paced acute medical/surgical unit to develop timely and appropriate treatment and discharge plans. Provides services to Veterans with highly complex health and/or mental health problems who may be in medical or mental health crisis. Provides social work triage assessment, crisis intervention, brief counseling, and rapid referrals to VA and community resources. Provides discharge planning interventions including to homeless shelters or to return to origin. Assists the inpatient medical team in setting up Renal services in the communities in which a Veteran resides and makes recommendations for needs upon discharge. Provides comprehensive psychosocial assessments and facilitates advance directive discussions and long-term care discussions.
