| This position performs a variety of therapeutic and program related activities associated with assisting Veterans with identifying goals, developing behavior plans, and taking action steps to increase adherence to treatment for the purpose of enhancing wellness and assisting with management of chronic disease. Assists Veterans with weight management, smoking cessation, addressing problematic areas in the eight components of proactive health and wellbeing. The position works independently or as part of a PACT team in assisting veterans in inpatient and/or outpatient settings. Health and wellness coaches do not diagnose conditions, prescribe treatments, or provide psychotherapeutic interventions; rather, they provide expert guidance and may offer resources from nationally recognized authorities. As partners and facilitators, health and wellness coaches support their clients in achieving health goals and behavioral change based on their clients' own goals and consistent with treatment plans as prescribed by individual clients' professional health care providers. Coaches assist clients to use their insight, personal strengths and resources, goal setting, action steps and accountability toward healthy lifestyle change. The position is appropriately assigned to the Social Science Series, GS-0101. This series covers positions the duties of which are to advise on, administer, supervise or perform research or other professional and scientific work in one or any combination of the social sciences when such work is not classifiable in other series of this occupational group. OPM has not specified titles for positions in that series. The title of Social Science Program Specialist is deemed most appropriate based on the work of the position. The position reviews current health behaviors and develop customized treatment plans with Veterans. These plans include personalized goals, action steps and interventions. The position helps the client utilize existing skills and develop new skills to enhance success. Schedules ongoing sessions to assess progress and assist in navigating obstacles or challenges. Guides Veterans to learn about available support tools in the VA and the community that assist in attainment of goals. Coordinates with others on the Veteran's health care team to ensure optimal patient care. Provides health coaching for Veterans in order to reduce or eliminate high-risk behaviors while increasing healthy ones. Conducts interventions for Veterans using one-on-one, couple, or group modalities in outpatient and inpatient settings. Conducts Whole Health Facilitated Groups; participate in MOVE, smoking cessation, diabetes management, hypertension clinics, mental health, and other inpatient and outpatient groups as appropriate. Provides follow-up care through telephone clinics or secure messaging between face-to-face appointments. Actively guides Veterans in the exploration of core elements of personal health planning through individual and group based programs. Provides eligible Veterans and family members with information regarding lifestyle management including weight management, nutrition, exercise/fitness, smoking cessation, stress management, diabetes, pain management, and other appropriate conditions that are in line with the personalized health plan developed between the health care team and the patient, and in coordination with programs providing specific services when applicable, e.g., MOVE! for weight management or smoking cessation classes, as necessary. Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met.
