This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Learn more The VA Northern California Health Care system is currently recruiting for a part time (20 hours per week) GS-12 Social Worker (Program Coordinator) for the Social Work Service in the Nephrology program. Major duties include, but are not limited to: Coordinates with community dialysis centers and providers and veteran's pact teams to coordinate care of the patient. Coordinates with inpatient team for discharge planning of the dialysis patient. Refers patients and families to a wide range of services (both community and VA), including providers of health care coverage, income maintenance programs, transportation services, vocational rehabilitation services, exercise programs, volunteer opportunities, local, state and federal agencies, etc. and helps patients and families utilize these services. Provides education regarding Transplants and the evaluation process. Tracks the results of each kidney and kidney/pancreas transplant center referral. Monitors the status of any patients on a Transplant waiting list. Communicates with the Transplant center regarding patient Transplant status quarterly and when there is a change in a patient's condition. Enters patients' Transplant status information in the MIS. Presents patients' Transplant status information at PCC's. Provides on-going comprehensive psychosocial assessment and counseling services to chronic kidney disease patients and their families to improve their quality of life. Provides case management as it focuses on both the patient's biopsychosocial status and the status of the larger social system. Conducts psychosocial needs assessment of the client, assists with arranging, monitoring, evaluating and advocating for a package of multiple services to meet the client's conditions. Initiates and/or participates in patient, family and staff orientation, education, rehabilitation and treatment programs such as developing educational materials and providing group services for patients and families and in-service programs for staff.
