| The Social Worker Sr. employs advanced practice methods in the development, implementation and evaluation of treatment plans while serving as a professional resource for patients and family members. The Social Worker Sr. coordinates a variety of services including crisis intervention, bereavement, client advocacy, referrals, prevention, education, discharge planning and implementation, and psychosocial assessments. The Social Worker Sr. collaborates in the development and implementation of performance improvement initiatives. The ideal candidate will have excellent verbal and written communications skills as well as listening and interpersonal skills.
