The GPD-Substance Use Disorder (SUD) Consult Coordinator is assigned to the Grant & Per Diem (GPD) program, as a part of the Homeless and Justice Clinical Recovery (HJCR) Program, within Mental Health Service at VA Salt Lake City Health Care System. Duties Include: Oversees and manages the admission process via the following activities: 1. Receives new referral requests (consults) via electronic record or direct contact from referral sources. Referrals arise from within the local VA system, the local community, the VISN, and national referral networks. Thus, excellent interpersonal and liaison skills are required. The GPD-SUD Consult Coordinator is able to coordinate with local services, including information and referral for additional resources from other VA, government, and community-based programs. In the course of such implementation, the GPD-SUD Consult Coordinator is expected to collaborate with these providers by supporting appropriate relationships to further VA objectives. 2. Evaluates substance use disorder treatment needs of new referrals, including assessment of American Society of Addiction Medicine (ASAM) criteria for treatment through chart review and direct interaction with referral sources. 3. Evaluates other mental health treatment needs through chart review and direct interaction with referral sources. This includes an emphasis on identification of treatment needs for persons who are experiencing homelessness. 4. Coordinates weekly with the SARRTP program and GPD Liaison regarding new candidates for admission and discussion about appropriateness for admission. 5. Acts as a subject matter expert related to SUD programming within the HJCR Program. 6. Identification and facilitation of referrals to other treatment providers or programs for individuals not deemed appropriate for admission to the GPD program. 7. Facilitation of admission to the GPD program for persons deemed appropriate. 8. Acts as a liaison between the VA SUD programs and HJCR programs. 9. Placement of persons on the GPD interest list if immediate admission is not feasible. 10. Maintains data and statistical compilations related to admissions and discharges to comply with VA and medical center policy and procedures, the Joint Commission, and the Commission on Accreditation of Rehabilitation Facilities (CARF). In coordination with the GPD Supervisor, will prepare reports and statistics for facility, VISN, or national use. 11. Has working knowledge and skill in the use of word-processing, data management and other computer systems, especially those programs in use by VHA. 12. Participate in workgroup and/or committee assignments, as deemed appropriate by the GPD Supervisor. 13. Communicates effectively, both orally and in writing, with people from varied backgrounds. Social Worker is able to meet the needs of customers while supporting VA mission. Consistently communicates and treats customers (Veterans, their representatives, visitors, and all VA staff) in a courteous, tactful, and respectful manner. The Social Worker is able to provide the customer with consistent information according to established policies and procedures and handles conflict and problems in dealing with the consumer constructively and appropriately. 14. Performs other related administrative duties as assigned. Direct Patient Care: 1. Provides a full range of social work services within commonly accepted standards of social work practice which includes case work, individual, family and group therapy, discharge planning, case management and other services as appropriate. 2. Independently conducts psychosocial assessments and formulate individualized treatment plans which include: clinical interventions to be employed, time frames, individuals responsible, and discharge planning. The Social Worker will develop treatment plans in collaboration with the Veteran and with the Veteran's treatment team. 3. Provides SUD specific treatment interventions to a caseload of Veterans in the GPD program. 4. Understand and possess the skills to: identify and respond to unsafe situations, facilitate crisis intervention assistance, mediate conflict resolution, and/or make the necessary referrals to ensure a safe outcome. 5. Meets individually with each Veteran assigned to their caseload to provide psychotherapy services, evaluate treatment plan progress, and implement changes as needed. 6. Meets with other staff and/or treatment team members daily to review each individual. 7. Properly documenting through electronic progress notes is essential to ensure quality control and completion of performance measures. 8. Provides or will receive training on how to deliver Evidence Based Therapy for SUDs, such as Cognitive-Behavioral Therapy for SUD and Motivational Interviewing. 9. Coordinates treatment and communicates with the care lines to facilitate continuity of care for Veterans. 10. Provides clinical supervision to students, interns, and less experienced staff in a way that promotes professional development and excellent service delivery.
