The Central Texas Health Care System is recruiting for a highly qualified Social Worker (Program Coordinator) to serve as the Women's Recovery in Supportive Environment (RISE) Admission Coordinator in the Social Work Service located in Waco, Texas. The incumbent is a professional social worker assigned to the Women's Recovery in Supportive Environment (RISE) Program. The incumbent works at an advanced practice level as an admission coordinator and senior social worker in the RISE. As admission coordinator, the incumbent is responsible for coordination of the referral and screening processes for Veterans seeking admission to the RISE and ensures timely and appropriate referral and notification process. The incumbent is the point of contact regarding inquiries for admissions to the RISE; maintains contact with Veterans and referral sources to obtain detailed information for decision-making purposes regarding admissions; attends the One Screening Committee for residential services on a daily basis; and schedules admission dates as deemed appropriate. The incumbent also enters data on the Bed Management System (BMS) for Veterans accepted for admission and maintains weekly contact with Veterans who are on the waiting list for admission. The incumbent collaborates with the clinical team to ensure program census is maintained and to address referral, screening, and admission issues. The incumbent is responsible for data management and tracking of referrals and admissions to the RISE. In addition, the incumbent may assist with provision of clinical social work services in the RISE. In this capacity, the incumbent must use a high level of skill in assessing and treating the complicated psychosocial problems of Veterans as they transition from the RISE to appropriate discharge alternatives. Care management responsibilities include the management, coordination, and provision of social work and supportive services to Veterans and/or their families, including provision of individual and group therapy to Veterans for issues related to Military Sexual Trauma, Substance Use Disorders, general Mental Health and other complex and diverse psychosocial problems. The incumbent also provides supportive therapy to family members. The incumbent is a member of an interdisciplinary team and is responsible for active participation in interdisciplinary treatment team meetings and resident centered care planning.
