This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Learn more The New Mexico Veterans Affairs Health Care System (NMVAHCS) Social Work Service (SWS) is looking for 1 Social Worker who will be assigned to the Medical/Surgical area in NMVAHCS and practices in the inpatient medicine units. They will provide coverage to inpatient surgery and other medical social work program area as needed. The focus of the social worker role is to provide care continuity between the acute setting and community-based services. Major duties include, but are not limited to: Independently conducts psychosocial assessments. Develops treatment plans in collaboration with the veteran/family and with the interdisciplinary treatment team with specific attention to culture, gender, ethnicity, sexual orientation, stage of life and age-related issues. Provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members. Advocates for and respects the right to self-determination including education related to Advanced Directives and end of life planning. Provides substance abuse, tobacco use, depression, and PTSD screening. Provides case management services to veterans, their families and significant others, including but not limited to, information and referrals, financial and benefits counseling, resource education, linkage to tangible service, and advocacy.
