The Social Worker in this position will report to the Health Care for Homeless Veterans Programs Supervisor, in the Social Work Executive Service Line at the Fargo VA Health Care System. They are responsible for outreach services to homeless veterans in communities and rural areas outside the main Medical Center campus and will operate separately from the main facility. Responsibilities for this position are performed in a broad multi-community and rural setting. Duties of this position include, but are not limited to, the following: Plans, develops and maintains a network of relationships with community resources and other advocates for the homeless to effectively screen and identify veteran members of the homeless population. Uses accepted social work theory and methods n the treatment of psychosocial dysfunction and depending upon need the Outreach Social Worker makes VA or community based provision of services, referrals, advocates for scarce resources, confirms referrals are completed and provides intensive follow-up and coordination with these agencies. Interviews potential clients confirms eligibility and attempts to complete a psychosocial assessment and make a psychological diagnosis as appropriate. Interprets and explains the VA's treatment and benefit programs. Clinically assess, participate in diagnosis and the development an appropriate treatment plan using known available resources and information from the psychosocial assessment. Clinical diagnosing using DSM-5 and ICD-10 behavior codes - Incumbent demonstrates knowledge in DSM-5 diagnosing and documents diagnostic impressions in psychosocial assessments when appropriate. Clinician may function independently or within a team to determine an appropriate diagnosis and formulate a treatment plan based on that diagnosis. Provide and document the patient's ongoing care in accordance with Medical Center policy and NASW and state Board of Social Work standards. Establish an ongoing clinical relationship with the client, evaluating progress, performing follow-up after referral and continued assessments to adjust treatment plans as needed. Present Veterans needs and progress to an Interdisciplinary Team, and advocate for the veteran's worth and dignity in the medical process. *** Recruitment Incentives may be authorized for highly qualified candidates.
