The incumbent manages administrative and oversight of the clinical aspects of the Contract Adult Day Health Care (CADHC) program. The incumbent is the supervisor for and manages the daily operation of various community based programs, develops policies and procedures for program operation of various community based and prepares reports and statistics for service line, facility, VISN, and national use. The incumbent utilizes funding data related to the program regarding day to day functioning, program evaluation and has responsibility for coordinating weekly interdisciplinary team meetings for CADHC and other programs. DUTIES INCLUDE, BUT NOT LIMITED TO: Provides administrative, technical, and educational consultation necessary for accomplishing the work of the program. Incumbent serves as the Coordinator and Contracting Officers Representative (COR) for the Contract Adult Day Health Care (CADHC) programs. Responsible for assuring that monthly follow-up to veterans in the CADHC are completed as required by VA Handbook 1147. Responsibility for coordinating weekly interdisciplinary team meetings and coordination and scheduling of annual inspections of the Contract Adult Day Health Care (CADHC) programs. Provide direct clinical services, and resolve problems that arise regarding patient care issues and requires specialized knowledge of issues related to community based services Has experience that demonstrates possession of advanced practice skills and judgment and at least two years in an area of specialized social work practice. Provides administrative, technical, and educational consultation necessary for accomplishing the work of the program, including education to health care system staff across the VA St Louis Health Care System and outreach efforts to community programs such as the state ombudsman program. Responsible for the Contract Adult Day Health Care (CADHC) programs reports to VISN and VACO for the coordinated programs and seeks guidance from VACO program office as needed. Delegated the responsibility and authority for the planning and coordination of all elements which constitute these community programs. Provide guidelines as to the general policy and direction of the Contract Adult Day Health Care (CADHC) programs.
