The New Mexico Veterans Affairs Health Care System is looking for (1) Senior Social Worker (Admissions Coordinator) to join the Behavioral Health Residential Division. The incumbent will serve as the coordinator of the screening and admissions process for Veterans seeking Residential treatment for Substance Use Disorders and/or Mental Health needs. The incumbent performs duties independently and reports directly to the Social Work Supervisor of the Residential Rehabilitation Treatment Program. Major duties include, but are not limited to: Arranges for admissions while tracking bed availability, transportation and need for medical support for withdrawal, and problem solves barriers to admission. Maintains at least weekly clinical contact with Veterans who have been screened and found appropriate for Residential care. Identifies systemic and procedural barriers to timely and appropriate patient access to care. Oversees, coordinates and participates in the clinical work of screening new referrals by phone within 24 hours of receipt of consult. Provides individual or group interventions to Mental Health Residential Rehabilitation Treatment Program patients as needed.
