This position is located in the Geriatrics and Extended Care Service at the Central Arkansas Veterans Health Care System in North Little Rock, AR. This VA Medical Center provides healthcare services to over 70,000 Veterans throughout most of Arkansas. CAVHS consists of two healthcare campuses located at Little Rock and North Little Rock, Arkansas, as well as 8 Community Based Outpatient Clinics (CBOC) throughout the state. The duties of a social worker in the Geriatrics and Extended Care Service include but are not limited to: Possesses a high level of skill and expertise to establish and maintain effective therapeutic relationships with Veterans/support systems. The incumbent independently works with Veterans/support system who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, financial, and psychosocial issues. Independently identifies high-risk needs and follows VHA integrative case management standards of practice, in order to respond both effectively and consistently to the whole health care needs of the Veteran, regardless of care setting. Independently completes an appropriate (brief or comprehensive) psychosocial assessment to determine the psychosocial functioning and needs of Veterans and/or their support systems. The incumbent utilizes the assessment in facilitating the Veteran's action plan to maximum use of treatment programs/resources for attainment of the highest feasible level of independence that is practicable. Actively participates and collaborates with the Patient Aligned Care Team (PACT) members and with Veterans/support systems to implement patient centered care that is personalized, proactive and patient driven healthcare. Possesses a working knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures, including acute, chronic and traumatic illnesses, and medical terminology. Provides Advanced Directive education to Veterans/support system and assists completing the appropriate forms. Also assists the Interdisciplinary team with Identifying appropriate decision makers if Veteran is unable to make own decision (Next of Kin, HCPOA, Legal Guardian). Facilitates advanced care planning group visits with Veterans and support system (as appropriate). Collaborates with Veterans/support systems and the Interdisciplinary treatment team members to ensure that appropriate VA and Community placements/referrals are completed in a timely manner. VA and Community placements or referrals can Involve, but are not limited to, referrals to contract nursing homes, Community Living Centers, community nursing homes, residential care homes, assisted living facilities, inpatient and outpatient hospice services. Possesses knowledge and ability to Independently implement appropriate Interventions, facilitate psycho-educational classes, and supportive groups for Veterans/support systems. Provides consultation and education to Veterans/support systems regarding community resources, VA benefits and specialty programs, and advance directives. This includes the incumbent maintaining knowledge of the process for accessing and/or coordinating community-based services, including information and referral for additional services from other VA programs, other government programs, and community programs. Provides consultation to other treatment team and staff members regarding psychosocial needs of Veterans/support systems and the impact of the identified psychosocial problems on the Veteran's healthcare planning and compliance with treatment. Possesses skills and knowledge to provide case management services to Veterans and their families throughout the continuum of care. The Incumbent will be able to appropriately utilize principles of human growth and development over the life span and will be able to assist Veterans in coping with the loss and grief experiences from disability and terminal illness. Possesses and demonstrates ability to communicate effectively, both orally and in writing, with people from varied backgrounds. Complies with established Social Work Service documentation standards. Supports the mission, policies, and procedures of the Department of Veterans Affairs, the Veterans Health Administration (VHA), the appropriate Veterans Integrated Service Network (VISN), and the facility. Can actively participate on committees, work groups, and task forces at the facility as deemed appropriate by the supervisor, Assistant Chief or Chief of Social Work Service. Keeps their supervisor apprised of problems and recommended solutions to problems encountered In the incumbent's area of responsibility. Ensures professional growth through continuing education by complying with licensure requirements and continuing education as appropriate to the area of assignment. Completes all Mandatory Talent Management System (TMS) and Basic Life Support (BLS) within required timeframes. Assists with the orientation and mentoring of newly hired social workers and, serves as a field instructor for social work students, if interested and qualified. Maintains a level of productivity and quality consistent with the complexity of the assignment and consistent with Social Work, Joint Commission. CARF and facility standards.
