The Inpatient Mental Health (IMH) Social Worker serves as an independent clinical social worker within an interdisciplinary treatment team within the Inpatient Mental Health clinic. The majority of services are provided in the inpatient setting with some outpatient and aftercare services provided as indicated. Services provided include biopsychosocial assessment and diagnosis, treatment planning, individual, group and family therapy, case management, advocacy, discharge planning, and coordination of treatment with other VA or community service providers or agencies as needed. Major Duties: Develops psychosocial assessment of the Veteran in collaboration with the interdisciplinary treatment team, the Veteran, family members and significant others. In coordination with the interdisciplinary treatment team will participate in developing a mutually agreeable treatment plan with the Veteran and family (when possible), to include Veteran-centered goals, objectives and interventions. Acts as the subject matter expert on VA and/or community resources. Responsible for maintaining and using a current social work resource file of VA and community services and programs and enhancing the content to the benefit of Veteran's. Responsible for making rapid assessments and developing crisis management for admission to mental health inpatient programs.
