Suicide Prevention Coordinator -Social Work Case Manager
The Suicide Prevention Case Manager (SPCM) will serve, in conjunction with the facility Suicide Prevention Coordinator (SPC), to coordinate the care to facilitate implementation of suicide prevention strategies throughout the Corporal Michael J. Crescenz Veterans Affairs Medical Center (CMCVAMC). The SPCM tracks high-risk Veterans and monitors treatment to insure that needed services are being received. Participates as a member of the hospital-wide Suicide Prevention Committee, helping to communicate daily operations of the management of patients at high risk for suicide to the committee and advocate for effective implementation of assessment of treatment of such patients throughout the medical center. Manages responses to alerts from the Veterans Crisis Line and helps to connect patients
