Social Worker- Behavioral Recovery Outreach Team Coordinator
The Incumbent has broad program management and clinical responsibilities for the Behavioral Recovery Outreach Team. S/he provides administrative oversight of the program to include planning, establishing, and implementing policies and procedures; developing program goals and objectives; and monitoring, operating, and evaluating the functioning of the program. S/he serves as a member of an interdisciplinary team and will provide direct social work/case management services to a panel of patients. Major duties include, but are not limited to the following: Clinical Responsibilities Exhibit a high level of skill and expertise in the work with patients and families who are experiencing a wide range of complicated mental, emotional, behavioral, physical, psychosocial, and environmental problems
