The Kansas City VA Medal Center is seeking a high qualified Social Worker that functions as a Case Manager for the Healthcare for Homeless Veterans (HCHV) Program. The HCHV Case Manager is responsible for providing outreach, assessment, referral and case management services to the homeless Veteran population in the community. To qualify for this position, applicants must meet both basic requirements and grade determination requirements by the closing date of this announcement.
