JOB SUMMARY The Administrative Director, Ambulatory Care Management, reports to the Vice President, System Care Management, with indirect reporting to ambulatory executive leadership and is responsible for leading and overseeing all Care Management functions within the ambulatory setting. This role will serve as an expert resource in ambulatory care management and provide leadership for the Ambulatory Care Management team, including nurse case managers, social workers, community health workers, and care management coordinators. This role requires an extensive background in the ambulatory setting including the critical thinking and decision making that arise during ambulatory patient care. Specifically, this role requires an extensive knowledge base of the con
