Summary:

Works under the director of the RN Case Manager and/or /Social Worker to complete referrals for post-acute services for patients and assists with tasks that do not require a clinical license or degree. Responsibilities include the following activities: arranges post-discharge services, creates and follows-up on electronic referrals using the Tenet Case Management system, reviews patient choice letters with patients/families for required signatures, provides follow up Important Message to Medicare patients prior to discharge, communicates with patient, families and other members of the care team, completes tasks assigned by Case Manager and Social Work staff, make copies, send faxes and completes phone calls, completes process reviews or audits as requested. Completes established competencies for the position within designated introductory period. Other related duties as assigned.

MINIMUM EDUCATION: High School diploma or equivalent
PREFERRED EDUCATION: Associate�s or Bachelor�s Degree from an accredited college
MINIMUM EXPERIENCE: 6 months in health care environment. Possesses excellent organizational, verbal/ written communication skills, problems solving and computer literacy skills. Data Analytics skills.
PREFERRED EXPERIENCE: Acute hospital experience
PREFERRED CERTIFICATIONS/LICENSURE: Paramedic, EMT or Nursing Assistant certifications.
REQUIRED COURSES/ COMPLETIONS (e.g., CPR): BLS-Obtained through approved American Heart Association Training Center or the Military Training Network. Non-Violent Crisis Intervention (NVCI) within 60 days of hire.
