General Position Summary As a member of the Trueblood Jail Diversion Program (TJDP) team, the Housing Coordinator helps clients identify goals that promote recovery, assists individuals who transition from Pierce County Jail into the community and provide supportive housing services. Individuals served by this program have experienced homelessness or are at risk for homelessness due to substantial mental, physical or behavioral health barriers that challenge their ability to maintain housing or live independently. Housing Coordinator will assist clients to achieve successful independent living, obtaining/maintaining housing, and pursuing vocational/educational goals. Provides outreach, housing search and acquisition assistance, independent living skills training, and assists clinical staff with crisis intervention and stabilization. Engagement skills are crucial in this position, as forensic clients often have a history of failing to engage with the mental health system or with other positive natural supports. This position works closely with the TJDP team, Jail Mental Health staff, Court Personnel, Pierce County Human Services, landlords and building managers, and other community providers. All services are provided under the supervision of a Mental Health Professional.
Responsibilities 1.Provides engagement services at the jail, coordinates housing plan with client and develops positive relationships with Trueblood clients. 2.Assists participants in developing successful independent living skills. 3.Assists participants in meeting recovery goals. 4.Completes clinical and administrative documentation in a thorough, accurate, timely and appropriate manner. 5.Completes progress notes on time for each service providing a clear, concise description of the clients progress towards treatment goals, being sure to document any risk concerns. 6.Practices effective caseload management in ways that meet productivity expectations.
Qualifications High school diploma/GED and 2 years of relevant experience. Associates degree preferred. Experience working with chronically homeless individuals with mental illness preferred. Must be eligible for a Washington State Department of Health Agency Affiliated Counselor Registration credential.
