The VA Portland Health Care System (VAPORHCS) provides extensive primary, secondary, tertiary, and quaternary care in outpatient, inpatient, residential, and community settings. VAPORHCS is a multi-site medical system that includes a major medical facility in Portland, Oregon, outpatient and residential facilities in Vancouver, Washington, and six Community Based Outpatient Clinics. To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 08/08/2022. Basic Requirements: United States Citizenship: Be a citizen of the United States. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education - Have a master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Grade Determinations, GS-12: Licensure - Licensed or certified by a state at the advanced practice level. Experience - Experience must demonstrate possession of advanced practice skills and judgment. At least two years should be in an area of specialized social work practice. May have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management. (b) Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice. (c) Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes. (d) Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills. (e) Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area. ["The HCHV Outreach Coordinator is responsible for providing clinical care coordination and oversight of all VA involved community-based outreach events. Oversight includes overseeing enrollment into the needed programs and engaging in community outreach and developing community partnerships including in rural settings. The incumbent will establish and maintain processes for referrals, evaluations, and admissions to various VA programs. The incumbent will facilitate regular complex clinical case reviews. The incumbent will conduct appropriate program audits ensuring compliance with all mandatory reporting requirements. The incumbent will assist the Director of CRS with preparation for and compliance with all accreditation agencies associated with this program, including Joint Commission FUNCTIONAL STATEMENT - HCHV SOCIAL WORK COORDINATOR, GS-0185-12 - MENTAL HEALTH: CRS 2 and The Commission on Accreditation of Rehabilitation Facilities (CARF). HCHV Coordinator will consult and negotiate with appropriate disciplines to meet Veteran and program goals and will monitor progress and adjust as needed. The HCHV Outreach Coordinator performs case finding functions and outreach, identifying and screening Veterans for psychosocial needs. Incumbent has advanced knowledge and expert skills and has the ability to implement treatment modalities, and provide counseling or psychotherapy for individuals, families, and groups. HCHV Coordinator writes practice guidelines for specific Continuum of Care areas to which coordinator is assigned that provides guidance to VA staff about community resources, Coordinated Entry processes ensuring that the most vulnerable homeless Veterans are served. The HCHV Outreach Coordinator identifies high-risk patients and provides case management services. Incumbent provides crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences and serves as liaison to family members. Has working knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology.
