The Senior Social Worker assigned to the CC&ICM model of care is responsible for building networks, case identification through evaluation and data analysis, direct case management services, facilitation of the Care Coordination Review Committee (CCRT), and direct collaboration with nursing and social work CC&ICM leads. The health care and psychosocial needs of the population served are complex and require a high degree of independence, clinical oversight, and creative problem solving.
