The Health Care for Homeless Veteran (HCHV) Coordinator is responsible for providing Veteran outreach through walk in clinics, at the Community Resource and Referral Center, and at multiple community sites of care throughout the VA Portland Health Care System catchment area. The HCHV Coordinator is also responsible for helping to coordinate and attend Homeless Program Outreach events throughout the year.
