This position will travel between Reunion Family Medicine and the Brighton Family Medicine Clinics.

Your day.
As a Behavioral Health Provider you need to know how to:
- Work with an integrated team of primary care physicians and providers to help with outpatient family practice behavioral health needs
- Provide comprehensive intake examinations and behavioral health assessments, and determines accurate DSM-5/ICD-10 diagnoses based on clinical information
- Provide behavioral health services to individuals, couples, families and groups, using a variety of treatment approaches and tools
- Make appropriate referrals for patients needing services outside the scope of the immediacy of primary care
- Maintain and complete timely clinical records on all clients
- Provide consultation to other staff members as requested and participates in staffing of cases
- Provide consultation and education services to individuals, agencies, institutions and groups as assigned
- Participate in staff training and attends staff meetings
- Carry out the assessment of individuals at high risk for dangerousness and implementing crisis management as needed
- Make referrals to a variety of social services and medical resources as needed
- Participate in the screening process of new referrals to the department as needed
- Promote the mission, vision and values of SCL Health while upholding service behavior standards.
Your experience.
We hire people, not resumes. But we also expect excellence, which is why we require:
- Masterâ™s degree in Social Work required
- Current LCSW, required
- Two years of clinical experience in related field is required
- Three years of clinical experience, preferred
- Previous experience in a primary care or integrated health team setting, preferred
- Bi-lingual in English and Spanish, preferred
