At Centura Health, we value incredible people with incredible skills - but your commitment to a greater cause is something we value even more.   Plans, implements and organizes social work resources and evaluates and provides treatment regarding mental health issues for patients; linkage to resources in the communityWhat You Bring/Job Requirements:In addition to bringing your whole self to the workplace each day, qualified candidates will need the following:   * Master of Social Work (MSW)* 6 months home care/hospice experience preferred* Two years post-Master's experience in health care or related field preferred* Licensed Clinical Social Worker (LCSW) preferred* Cardiopulmonary Resuscitation (CPR) required, Basic Life Support (BLS) from American Heart Association required for facilities with an Automated External Defibrillator (AED) preferred for facilities without an AED* Current Driver's license Physical Requirements- Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10lbs. constantly This is for a PRN day shift (8-5pm).  We do rotate coverage on holidays.   We are patient and family focused with a highly collaborative team.  We have heavy volumes with the flu season.  Typically we will see mostly adult patiens with some NICU patients.   Your Connected Community:When people and places align, incredible things happen.
