This position is located at the Albany VA Medical Center in Albany, NY. The Social Worker will serve as part of the Mental Health and Social Work Service assigned to the Primary Care Mental Health Integration (PCMHI) Program. The Social Worker will function as a Collaborative Care Manager delivering targeted behavioral interventions as well as other protocol-driven services. Applicants pending the completion of educational, or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
