Summary:

This position oversees the daily management of the Care Management Department. The position is responsible for providing expertise and leadership to the Care Management team to insure the effective utilization of resources for patient care delivery and administrative staff. This position will also enhance the continuity of care and cost effectively by integrating the functions of case management, utilization management and discharge planning.

CHRISTUS Santa Rosa Hospital - Westover Hills (CSRH-WH) is a 150-bed hospital serving the fastest growing area of San Antonio. Specialized care includes orthopedic and surgical services, ICU, women's services, a newborn nursery, comprehensive cardiovascular care from diagnostics to open heart surgery, vascular lab, sleep center, emergency services, the CHRISTUS Weight Loss Institute, wound care, rehabilitation, and more. The campus also boasts an Outpatient Imaging Center and three medical plazas, one of which houses our CHRISTUS Santa Rosa Family Medicine Residency Program and CHRISTUS Santa Rosa Family Health Center.

Requirements:

• Bachelor's Degree in Nursing, Healthcare Administration or related field required.
• A minimum of 5 years in clinical practice for population to be served is required.
  
• Case Management/Utilization management and discharge planning experience preferred
• Licensed as RN in the State of Texas (applicable if RN).
  
• Word processing in a windows environment and Power Point presentations
  
• Ability to analyze and create reports and presentations via software applications
• Familiarity with ACCESS preferred
• Ability to communicate with other members of the team
• Bilingual (English - Spanish) skills preferred.
• Works in a clean well-lighted environment.
  
• Hours will be long. Will be called in or scheduled for weekend or evening activities as needed.
• Regular local travel to appointments and frequent trips throughout hospital are required.
• Must also successfully complete the pre-employment/post job offer health screening examination, and the annual screening each year thereafter as an employee.
