The Supervisory PACT (Patient Aligned Care Team) Social Worker serves in combined clinical and supervisory roles within the PACT/Primary Care areas at the Ralph H. Johnson VA Medical Center in Charleston, South Carolina. The Patient Aligned Care Team is a comprehensive team which delivers primary care to Veteran patients with a focus on prevention, health promotion and wellness, care coordination and chronic disease management. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
