The VA Northeast Ohio Healthcare System is recruiting for a Social Worker. The Social Worker will function in Home and Community Based Care. The assignments to these social workers are multi-faceted and may be in the Community Nursing Home, Community Adult Day Health Care, Community Respite, Community Hospice or Homemaker/Home Health Aide Programs. A number of these patients are in several of these programs at the same time which requires additional oversight for coordination purposes. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
