The Outreach Social Worker is assigned to the Veterans Affairs (VA) Medical Center and provides services to patients/significant others treated in the Homeless Veteran Program. The social worker recognizes the age-related needs and demonstrates knowledge and care appropriate to that age group. The Social Worker provides outreach, assessments, referrals and case management services to homeless Veterans identified in the community, Clinic, Hotline Calls, consults, and community shelters. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE).
