The HCHV Outreach Social Worker is a full time position assigned to the Health Care for Homeless Veterans (HCHV) Team aligned the Mental Health ICC at the Syracuse HCS. The HCHV Outreach Social Worker works closely with the HCHV Team and if based at a CBOC, may also meet regularly with the CBOC Behavioral Health BHIP Team. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
