Working closely with YRMC clinical staff, patients, and patient families, Social Workers provide psychosocial assessments and treatment recommendations as well as conducts discharge planning. This individual works with the patient and his / her family to cope with the patient's hospitalization, illness, diagnosis, treatment, and/or life situation, including emotional, mental, and substance abuse disorders. The YRMC Social Worker works with a wide range of patients ranging from pediatric to geriatric populations.

Summary:

The Social Worker provides psychosocial assessments and treatment recommendations, as well as discharge planning to and consultation about patients and families to assist them and the health care team in coping with patient's hospitalization, illness, diagnosis, treatment, and/or life situation, including emotional, mental, and substance abuse disorders; facilitates transfers to other acute care hospital settings or skilled nursing facilities.
