Transition Management Manager- Inpatient

Position Summary:
The Inpatient Manager leads 30+ front line staff members of the Transition Management department and actively participates in supporting management functions including staff development and evaluation of clinical practice. The Manager develops and maintains standards of practice in accordance with evolving practice of case management, social work, and utilization management. Trends and reports avoidable days, RLOS, readmission and other metrics to ensure optimal performance. Effectively utilizes department Physician Advisors and Medical Directors (TMO & UM) to intervene when continued stay criteria are not met. Serves as resource to facilitate complex cases and escalate concerns and feedback to department Director/Vice President.
Department Summary:
Vanderbilt University Medical Center (VUMC) is accepting applications for the Inpatient Manager role within our Transition Management Office (TMO). The TMO administrative office is located in Medical Center North, on campus serving VUMC. Transition management or commonly referred to as care coordination is a thriving department comprised of nurses, social workers, and support personnel. Currently there are 150 staff however the department has increased their staff needs to enhance the patient experience at Vanderbilt. The Vice President of TMO is Marcia Colone, Ph.D. who has over 25 years of experience in care management. Dr. Colone leads a team of experienced Directors and Team Leads who mentor, coach, and support staff within the coordination team.
Additional Key Elements/ Responsibilities:
- Ensures the department's optimal performance among their team by engaging in effective problem solving with inter professional staff, clinicians, patients and families.
- Responsible for team accountability to performance standards including quality, service, patient flow, communication and credo behavior. Interacts effectively with members of the health care team and ancillary departments to monitor and achieve quality patient outcomes.
- Supports the delivery of quality patient care by holding staff accountable to established patient care and regulatory standards.
