The incumbent carries responsibility for crisis intervention and case management on a short- term basis in collaboration with the veteran's care provider(s) while a veteran is at high acute risk for suicide. The incumbent assesses the psychosocial and environmental needs or dysfunction secondary to or exacerbating the psychiatric problems which might contribute to the risk of suicide. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. High Risk Flag Maintenance: The incumbent is responsible for tracking the care of veterans with the Category I High Risk for Suicide Patient Record Flag assigned to their medical records. The incumbent will ensure each flag is activated, reviewed, and inactivated within the time frames designated by the National Office for Suicide Prevention. REACH VET Maintenance: The incumbent is responsible to assist in the REACH VET Coordinator in all major components of the program when requested. This includes using the dashboard to track identified patients, inform providers of their identified patients, complete required documentation in CPRS, conduct outreach attempts and transfer/receive transfer of patients in the program. Treatment Planning/Goal Setting: The incumbent is responsible for collaborating with the treatment team and setting achievable treatment goals with the veteran/family. Treatment/Therapy: The incumbent is responsible for providing appropriate short-term treatment for a variety of mental health disorders. The incumbent may see veterans to supplement or compliment current and ongoing treatment plans. Case Management: The incumbent is responsible to assess, plan, implement, coordinate, monitor and evaluate the options and services required to meet the needs of veterans at high risk for suicide. The incumbent will work effectively on behalf of the veteran both within the VA Health Care System and outside agencies to provide cohesive and comprehensive care. Will ensure appropriate follow up on missed appointments for high-risk patients to ensure patient safety. Referral to service providers: Throughout the course of treatment the incumbent is the subject matter expert on Suicide, VA Health Care System and/or community resources.
