The incumbent is a Social Work Program Coordinator and reports to the Social Work Executive. This Social Work Program Coordinator falls under the Social Work Executive, and collaborates with the Chief of Long Term Care. The incumbent is responsible for the professional and administrative management of the Veteran Directed Care (VDC) program in the Martinsburg, WV, catchment area. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Citizenship. Be a citizen of the United States. Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure. Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. ? 7403(f). Grade Determinations: In addition to Basic Requirements the below is required Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice Preferred Experience: Candidate will preferably have experience with developing and coordinating the management of a new program. As the Coordinator, the incumbent assists in planning, establishing, and implementing policies and procedures; developing program goals and objectives as well as monitoring, operating, evaluating, coordinating and daily operations of the assigned programs to ensure Veteran access to purchased home and community service. Responsibilities include liaison with community providers, identification of community resources, staff education on program services, referral processes, and the development, evaluation and communication of standards, policies and procedures. In addition, the incumbent will uses sound judgment in the interpretation of trends and patterns; the application of guidelines; and the recommendation of program enhancements. Program Coordination - As an advanced practitioner, incumbent serves as the Social Work Program Coordinator of the VDC program. Incumbent duties include all administrative oversight and clinical responsibility for all programs under responsibility. Incumbent is responsible for weekly, monthly, and quarterly meetings and reports respective to the assigned program. Incumbent is also responsible for program reports to VISN and VACO. Seeks guidance from the VISN and VACO program office as needed. Develops and implements information systems to track service activities including visits, referrals, patient demographics, and patient satisfaction. Duties include ensuring Medical Center policy compliance with the VDC program. As the Coordinator, incumbent is responsible for scheduling, organizing and inspecting of community contracted providers, and completion of appropriate required reports and follow-up to ensure Veterans being provided contracted care are receiving appropriate care. Incumbent is responsible for negotiating, facilitating and completing basic ordering agreements/contracts for the VDC. Incumbent is responsible for the development, coordination, selection and evaluation of VDC in the Martinsburg catchment area; implementation and application of VA guidelines to ensure standards maintained promote health, safety, welfare, and satisfaction of the Veteran receiving community services; coordination of annual inspections and appropriate oversight to ensure compliance with all VA guidelines; assurance of optimal qualitative/quantitative clinical program services via interdisciplinary collaboration, program development, and effective utilization of VA/community resources to provide appropriate services and patient advocacy.
