Specially designated HUD funding for the Housing Choice Voucher (HCV) program provides rental assistance under a supported housing program for homeless Veterans. Known as the HUD-VASH program, it combines Section 8 rental assistance with case management and clinical services provided by the Department of Veterans Affairs (VA) at its medical centers and in the community clinics. To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 08/07/2023. Basic Requirements: United States Citizenship. Education. Possess a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. For grades below or at the full performance level, VHA social workers who are not licensed or certified at the time of appointment must become licensed or certified at the independent master's level within 3 years of their appointment as a social worker. Licensure. Possess a full and unrestricted license or certification by a state to independently practice social work at the master's degree level. English Language Proficiency. The social worker is responsible for providing clinical case management and clinical services for a caseload of Veterans in the Housing and Urban Development - Veterans Affairs Supportive Housing (HUD-VASH) program. The social worker is responsible for reviewing agreements between Veteran participants and landlords, Public Housing Authorities, and the VA in establishing appropriate referral sites for HUD-VASH placements. 2. The social worker performs site visits to HUD-VASH Veteran's apartments to ensure they are residing in safe environments in compliance with local housing codes. 3. The social worker verifies the Veteran's status, eligibility for VA medical care, and eligibility for program participation. 4. The social worker is responsible for ensuring homeless Veterans who meet criteria for chronic homelessness are considered as primary participants for program acceptance. 5. The social worker is responsible for collecting and submitting HUD-VASH Program participant data as outlined by NEPEC evaluation procedures. 6. The social worker is responsible for providing HUD-VASH clinical services. Clinical services are defined as the provision of services by VA clinical staff to homeless Veterans and includes all activities to provide appropriate treatment and maintain Veterans in HUD approved stable housing. 7. The social worker coordinates care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery and, linking and referring Veterans to the VA Medical Center, VA Regional Offices and/or community agencies. 8. The social worker complies with criminal conflict of interest laws and Executive Branch Standards of conduct to avoid conflicts of interest while serving as a HUD-VASH Clinician. 9. The social worker actively participates in HUD-VASH conference calls and attends training offered by the HUDVASH Program. 10. The social worker is responsible for following local policy and SOPs for the HUD-VASH Program. This includes following policies and documentation requirements as required for CARF and JCAHO accreditation. 11. The social worker actively participates in program planning and development, advising the HUD-VASH Supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences. 12. The social worker takes responsibility for improving their own professional skills, participating in planning, and executing specific staff development projects, attending seminars and institutes, and participating in training programs of other disciplines as requested. 13. The social worker performs accountability functions: 1) appropriately documents all contacts with patients/families in the electronic medical record; 2) completes statistical and other reports in accordance with VA Medical Center and respective professional discipline service policies. 14. The social worker works with VA Medical Center multi-disciplinary teams and community agencies to strive for developing more effective and efficient ways of delivering healthcare services to HUD-VASH Program veterans. 15. The social worker will conduct psychosocial assessments to determine needs of Veterans and family members. 16. The social worker is an active participant in the treatment planning process with other disciplines. 17. The social worker will participate in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate, with appropriate warm hand offs when transferring patient care. 18.
