The Care Coordination, Advocacy, Treatment, and Connections to Housing (CATCH) Social Worker will be based at Mather, CA in the Sacramento VAMC. The incumbent coordinates clinical and psychosocial services and is accountable for the overall effectiveness of the services provided, modifying services to best meet the Veteran-identified treatment needs and to promote efficient practice under the direction of a Supervisory Social Worker.
