The Social Worker, Home Based Primary Care (HBPC) Program Coordinator is a Social Worker who demonstrates advanced practice skills and judgment. The HBPC Program Coordinator is licensed by a state to independently practice at the Advanced Practice level and coordinates/directs the administrative and clinical aspects of a major specialty treatment program (Home Based Primary Care). Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
